Bekerja seperti tiada life…


Minggu pertama bekerja, kami masih lagi homeless, memang xde masa nak cari umah, so kami merempat di rumah encik azwadi di Taman JP Perdana (area dekat Hosp Sultan Ismail, arah ke Kota Tinggi kalu dari JB). 

Bertolak dari situ setiap pagi pukul 6.50 pagi, dan tiba di HSA pukul 7.45pagi. Pastu kena punch in kat bangunan X-ray dan bergegas ke In-Patient (Ward Supply) Pharmacy.

Kenapa bergegas? Sebab setiap pagi ada staff meeting kat unit ni. Apa yang dibincangkan dalam meeting? Biasanya update pasal stok, pekeliling terbaru, isu hari sebelumnya, ubat special untuk doktor-doktor pakar di wad etc…

Apa jobscope di In-Patient pharmacy? Klien kami adalah ward-ward di HSA, dan tugas kami adalah bekalkan ubat "floor stock", ubat "emergency trolley", dangerous drug (seperti morphine), psikotropik drug (seperti diazepam) dan memproses prescription bagi pesakit yang ditahan tinggal dalam wad.

Macam senang bunyi nyer kan? Actually tak pun… Dan banyak masalah yang timbul dalam proses-proses ni.

Mula-mula start keje kat unit ni, saya ditugaskan di bahagian “filling”. Kerja ni tak susah sangat la, cuma top-up floor stock setiap wad, bekalkan ubat emergency trolley dan bungkuskan ubat mengikut prescription yang telah di “screen” dan di label ke dalam troli ubat.

Tapi yang susahnya sebab tak tahu lokasi ubat time mula-mula kerja… Ada banyak giler ubat dalam rak-rak ubat, ada yang dalam bilik fridge, ada yang dalam stor je, ada yang kat bahagian ubat pakar, so struggle jugak la nak cari satu-satu ubat untuk masukkan dalam trolley ubat. PPF banyak membantu time awal-awal ni…

HSA mengamalkan  sistem pembekalan ubat secara Unit of Use dan Unit of Dose (tapi sikit je wad yang guna UOD ni, kebanyakan guna UOU).

Selepas sehari dua buat filling je, dah graduate kepada counter-checking… Buat filling dulu, bila dah siap, counter-check untuk pastikan ubat yang dah di bungkus tadi ubat yang betul, strength ubat yang betul dan kuantiti yang dibekalkan mencukupi. Biasanya countercheck ubat yang di fill oleh orang lain la, yang kita fill tadi seeloknya bagi orang lain check…

Awal minggu kedua, akhirnya graduate kepada tugas yang lebih mencabar, “screening” prescription. Prescription akan dihantar by wad dan perlu disaring. Tugas ni agak sukar untuk di terangkan, banyak prosesnya, tapi the jist is that kami akan pastikan ubat yang diprescribe oleh doktor adalah sesuai, dalam dose yang betul, tarikh betul, dan dibekalkan dalam kuantiti yang mencukupi sehingga trolley day yang berikutnya… Kalau tak, boleh reject prescription tu...

Penting semasa proses screening ni untuk mengetahui apa preparation yang ada di farmasi tu, sape yang boleh prescribe (tengok MOH blue book dan policy hospital), dan stability setiap IV preparation.

Minggu ketiga saya graduate lagi kepada tugas CDS pulak… Dalam tugas ni, saya kena key-in manual prescription yang dah disaring tadi ke dalam sistem komputer dan buat computerized nyer label for new prescription (yang bukan top-up prescription).. Tugas ni sangat mencabar sebab ada satu komputer je yang digunakan untuk CDS ni jadi sorang je yang akan buat tugas ni sepanjang minggu..

Kira 4-5 orang pegawai farmasi akan screen pastu bagi kat sorang untuk key-in dalam computer… kalau lambat buat memang obvious sangat la sebab nanti fail akan membukit di sebelah, dan FRP dan PPF pun akan tegur… hehehe… Neway, selepas dah key-in dan label, baru prescription akan di fill dan akhirnya di counter-check…

Apa lagi tugas-tugas di in-patient pharmacy? Jaga dangerous drug dan psikotropik drugs (saya belum buat lagi), buat extemporaneous preparation (biasanya syrup untuk paeds dan neonates wards), buat ward inspection, dan bedside counselling (naik ke ward dan ajar patient guna device cam inhaler dan eye ointment etc)…

Sepanjang minggu pertama, kami berempat punch kad pukul 5 lebih dan terus beli makanan di cafĂ© hospital… Keluar dari hospital pukul 5.30ptg tapi akhirnya sampai di rumah senior tu dalam pukul 7.15malam (waktu berbuka)… Malam pulak tido awal supaya esoknya bangun awal bersiap ke hospital… Memang takde life…

That concludes my experience in HSA In-patient Pharmacy (Induk)… Next week saya akan bertugas di Satellite in-patient pharmacy pulak… mungkin lebih mencabar… Semoga coretan yang ringkas tapi masih berjela2 ni berguna untuk orang yang membacanya… hehe

Semuanya bermula di sini…


It has been 2 weeks since I started working as a Pegawai Farmasi Provisional (PRP) at Hospital Sultanah Aminah (HSA), Johor Bahru… Life has been pretty hectic ever since… 

It started on the 2nd of August 2011 (because 1st August coincided with 1 Ramadhan, a public holiday in Johor) at the Wisma Persekutuan JB.

Here, we (8 of us from UIAM) were asked to lapor diri at the Jabatan Kesihatan Negeri Johor.

The process here takes just about 10 minutes, we were just asked to fill in a few forms and submit a few photocopied items. Here, I accidentally ran into Ain Yasin and Rose (my schoolmates). Both were there to lapor diri as housements.

Then we got going to Menara MSC Cyberport (a.k.a Menara Sarawak) to the Bahagian Farmasi Negeri Johor to know our penempatan. 

At first, I kinda hated the idea of having to lapor diri at 2 places (why not just 1 kan?), but after much thought, I think that at least this is the kind of autonomy that we should be proud of as pharmacists, our penempatan is not decided by the Jabatan Kesihatan Negeri, but by our very own Bahagian Farmasi. 
 
Neway, we were told to wait for all 8 of us to arrive before proceeding to the agenda. Miss Melissa Hong, the pharmacist in charge of PRP at Bahagian Farmasi listed out all the choices available for us, 5 posts at HSA, 2 posts at Hospital Segamat, and 1 post at Hospital Batu Pahat. We were asked to discuss and choose ourselves, but if fail to reach any agreement, it will be decided by drawing lots.

Azlen chose Hosp Batu Pahat, Farah Hani and Mariam – Hosp Segamat, Fatimah – HSA… So that left us 4 guys (me, topek, matto and gigi) with HSA… It was not where we expected to be posted but at least we got stuck together, takde la sorang-sorang nak cari rumah etc kan...

We were given letters addressed to our respective Pengarah Hospital and were asked to lapor diri at the hospitals as soon as possible (because gaji start kira dari hari lapor diri di hospital).

At HSA, we were directed to Puan Zainon, a staff at the office of Pengarah Hospital. Then it was time to fill in lots and lots of paperwork. 

Around 3pm, we (5 of us) made our way to lapor diri at Ketua Jabatan Farmasi HSA (a.k.a Ketua Pegawai Farmasi). We talked for a while and then we were passed on to another pharmacist to go to DIS unit.

There, we met, Puan Zarina (also the head of DIS), the one in charge of new PRPs. We were given a tour around all the different units in pharmacy department, followed by an orientation about holidays, work etc (I almost dozed off in exhaustion during this part).

Before we punched out for the day, we were assigned our first placement. Topek and me were assigned to In-patient (ward supply) pharmacy, Matto and gigi were assigned to Out-patient pharmacy while Fatimah was assigned to DIS unit. 

For the next 4 weeks, we would be expected to work our asses off in these units and then move on to other units to complete our rotation.

By the time we punched out, it was already 5.30pm. Exhausted, and still homeless, we decided to squat at Azwadi’s (our senior) house near Hospital Sultan Ismail (which is quite far from HSA). That wraps up my lapor diri day.

Mesti korang pening baca kena lapor diri kat banyak tempat (5 places) sebelum mula kerja, ya memang penat dan memeningkan. Hehe.
